Is your professional identification card (PIC) about to expire? Are you sick and tired of the long queues at PRC? Or just don't have an entire day to spend at PRC?
Well, fret no more! There is now another way of renewing your PIC 7 days a week and in just minutes. This innovation is the "PRC ID Renewal System at the Malls", part of PRC's e-Services, available at SM Malls.
Read on to find out more.
It has already been three (3) years when I got mine after passing the dreaded Philippine Nurse Licensure Examination and it's about time I process for renewal. Since I now work for the Foundation for Adolescent Development Inc., 5 days a week from 8AM to 5PM, I can't take a day off and go to PRC.
I took advantage of the abovementioned service and felt like blogging about it to share with you my experience. A worthy experience, I must say.
It was just yesterday afternoon when I decided to call SM City North EDSA Business Services Center and inquire about the process. I checked on the requirements needed, its fees, hours of operation and other stuff. You can just Google the contact information of the SM Mall nearest you and inquire the same.
But, since I enjoyed the experience, here is the step-by-step process once you're at the Business Center.
By the way, the Business Center at SM City North EDSA is located at the 2nd Floor Skybridge, The Block. It's right after SM Cinemas 5-6-7-8. Haha. If you're like me who used to live just behind SM North for a couple of years, no wonder you'll be familiarized with its ins and outs.
1. GO TO THE PRC RENEWAL LANE.
In the case of SM North EDSA, this is located on the left end side from the glass door. The teller would hand you a sheet of paper indicating the requirements as follows:
A. One (1) passport sized ID picture, close up, in white background and with nametag.
B. Professional must appear in person and must wear anything smart casual.
A polo shirt top and jeans will do. You'll know in a bit why you're required to wear such.
2. FILL UP THE APPLICATION FORM.
Since I already called prior to going there and since I came from the office today, I already have my picture ready and I'm donning my usual office wear - a polo shirt and jeans combination.
The teller would then hand you the Application Form for Professional Identification Card.
This is the same exact form you filled up during your initial registration. You may also download the form at the PRC website and print and submit to the teller. Moreover, take note of the important dates of your professional career - date of examination, registration date, and expiration date. You will need these to complete the application form. Also, take note of your license number. This I believe should be a no-brainer. I bet you have it memorized, from the day you get hold of your professional license.
Once finished, submit the form and the picture to the teller.
3. SMILE FOR THE CAMERA.
This is the reason why you're being asked to come in a smart casual attire. Your picture for your renewed card will be taken on the spot. You will be asked to stand in front of a white wall for background.
Hmmm! You might be wondering why as you already gave a passport sized ID picture. If you are to check the PRC website, they would require you to submit 2 pcs of your picture. Hence, the picture you submit will then be submitted to PRC for file.
Remember to don your best smile as this will be in your professional identification card for the next three years! :)
4. PAY THE CORRESPONDING FEES.
The usual price for renewal at PRC Offices is P450.00. SM Malls charge a fixed service charge of P50.00. You also have the option of picking up your PIC at the same Business Center or for just additional P80.00, you can have it delivered right at your doorstep.
5. WAIT FOR THE CLAIM STUB AND RECEIPTS.
Once your picture has been taken and you have paid the fees, the teller will give your claim stub and receipts. The claim stub would also have the Business Center number so you can follow up with them.
6. PICK UP YOUR PROFESSIONAL IDENTIFICATION CARD AFTER 20-30 WORKING DAYS.
Yes! You read it right. If you choose to pick up your PIC at the Business Center, it will take 20-30 business days, holidays and weekends excluded. It would also depend on how long does PRC delivers the same to the branch where you renewed it.
I am expecting mine to be released by the 2nd to last week of August!
This I can say is one of the major downside of the service. It would only take you 2 weeks if processed at PRC. Nonetheless, this service will save your time and spare you the hassle of long queues at PRC.
SM Business Centers are open 7 days a week, Mondays to Sundays from 10AM and the PRC Renewal Lane has a cut off time of 7PM.
I was at the Business Center by 6:30PM and went home feeling relieved from possible penalty charges of non-renewal before 6:35PM.
So, if you're just like me who don't have a day to go to PRC and not so in a hurry to get you're renewed PIC, I highly recommend you this service.
The "PRC ID Renewal System at the Malls" service was launched last August 6, 2012 and is continually gaining popularity among professionals wishing to renew their PICs at their own convenience.

DeleteIt was no longer approach in a span of 20-30 working days. The last time I went at SM Business Center at SM North, the teller advised me that I will be able to acquire my renewed license after 2 to 3 consecutive months. So, I suggest it would be finer to take care of it at the PRC office itself.

DeleteUnfortunately, SM Business Centers ONLY process "renewals" as of this time. Anything that has to do with changes, duplicates, reprints, change of name, etc, they would recommend that you process them directly thru PRC.

Sadly you will NOT be able to process your cousin's PRC ID renewal at SM Malls. They would require the professional, your cousin in this case, to be physically present as her picture will be taken right on the spot.
What I would recommend is that you go directly to PRC and process the renewal from there. I hope she also sent you her most recent ID picture (passport size, with nameplate and in white background) along with the authorization letter.
I'm leaving for the States on January to get married but I think its best if I wait 'til I get married before I renew my PRC license so that my representative would only need to go to PRC once to change the name and renew the license. What do you think?

DeleteYes! I would recommend you process your renewal once you're married so that the change in status and last name would be included. Please make sure that your representative have a Special Power of Attorney or a duly signed authorization letter once he/she goes to PRC. It would also be best if you leave with him/her a copy of any of your valid ID just for verification purposes. ^^

With regards to your concerns, to be honest, I am not sure of the "maximum" time allotted by PRC before they can consider a professional "inactive". However, upon checking RA 9173, I find this:

Section 23. Revocation and suspension of Certificate of Registration/Professional License and Cancellation of Special/Temporary Permit. - The Board shall have the power to revoke or suspend the certificate of registration/professional license or cancel the special/temporary permit of a nurse upon any of the following grounds:
(a) For any of the causes mentioned in the preceding section;
(b) For unprofessional and unethical conduct;
(c) For gross incompetence or serious ignorance;
(d) For malpractice or negligence in the practice of nursing;
(e) For the use of fraud, deceit, or false statements in obtaining a certificate of registration/professional license or a temporary/special permit;
(f) For violation of this Act, the rules and regulations, Code of Ethics for nurses and technical standards for nursing practice, policies of the Board and the Commission, or the conditions and limitations for the issuance of the temporarily/special permit; or
(g) For practicing his/her profession during his/her suspension from such practice;
Provided, however, That the suspension of the certificate of registration/professional license shall be for a period not to exceed four (4) years.

As for your representative here in the country, I believe that as long as he/she will bring a Special Power of Attorney or a duly signed authorization letter and as long as he/she can provide the Commission with valid identification cards, the request for renewal will be granted.
This info I got from the PRC website (www.prc.gov.ph) might help:
A. RENEWAL AT PRC CENTRAL AND REGIONAL OFFICES
1. Professional or duly authorized representative must secure and accomplish a renewal form from the Registration Division and submit one (1) passport size picture with nametag and in white background.
2. Professional pays to cashier the renewal fee upon securing his/her printed renewal application. The schedule for claiming the PIC is printed on the lower portion of the printed form.
3. On the scheduled date, the professional or his/her duly authorized representative must present claim stub to the Registration Division. He/she shall also present the Official Receipt showing payment of the renewal fee and sign an acknowledgment of receipt of the PIC. When the PIC is claimed by a representative, the said representative must also present together with the official receipt a *letter of authority or **Special Power of Attorney from the professional.

Hi! I'm afraid I don't have the exact amount as to how much the penalty is. I asked for this one when I renewed my license but the teller informed me that there she cannot give me any amount as the penalty is computed based on the number of months. So, technically, if I'm not mistaken, the formula is AMOUNT x # OF MONTHS.

What I would recommend is you can try visiting the nearest SM Mall with PRC ID Renewal Counter and ask for the amount of penalty you incurred.

Yes, you can still renew your license. I believe the provision for refresher course/s was implemented during the old nursing law (RA 7164) for those who fail to pass the NLE for three (3) consecutive times. However, there is no provision in the current law (RA 9173).
Moreover, according to Article V, Section 26.
"Requirement for Inactive Nurses Returning to Practice. - Nurses who have not actively practiced the profession for five (5) consecutive years are required to undergo one (1) month of didactic training and three (3) months of practicum. The Board shall accredit hospitals to conduct the said training program."

DeleteUnfortunately, SM Malls do not cater to changes in status for professional licenses as of yet. What I would recommend you to do is to go to PRC and process it from there.
As to the release, I have friends who renewed at PRC and they were able to get their renewed licenses on the same day. Though, I know of some who were asked to come back for the release of their licenses after 2 weeks.
I am just not so sure of the process though if it would entail changes on your records.

As far as I'm concerned, whether you renew your license on or before it expires, you are required to pay for the same renewal fee of P450. It would only vary if you renew your PRC ID after its expiration date.

I processed my application on 18 July and got it on 22 August. So basically it took me 25 days before I was able to claim my renewed PRC license. By the way, unlike PRC, you would need to personally claim your ID.
Here's an update, I believe that if you would now process at PRC, you would be able to get your renewed ID on the same day. You would only need to spend the whole day though!

To be honest, ngayon ko lang narinig ang "pakisuyo center" na naniningil ng P3,500. Masyadong malaki ang "patong" nila mula sa original na P450. At sa pagkakatanda ko rin, hindi kailangan ng certification na ikaw ay miyembro ng kahit anong nursing organization bago ka makapagrenew ng iyong lisensya.
Bago ka magbayad sa "pakisuyo center", subukan mo munang tawagan ang pinakamalapit na PRC Regional Office o magpunta ka sa pinakamalapit na SM Mall na may PRC Renewal Service.

I believe there's no need for you to secure an affidavit of loss. SM Business Centers would not require you to present your expired PRC ID. However, since your license expired June of last year, on top of the P500 renewal and processing fee, you would need to pay a penalty fee computed for the months that your license has already been expired.
Hi Sir... you might also have an answer to my query. I need to renew my expired license ID as well as to have my name and civil status changed from my married name back to my Maiden Name and as a Single person. This is by the way after the annulment case I have filed against my former husband was granted by the court. Can I process all these at the SM Business Center in Iloilo? What are the requirements and How much do I need to pay since my license has expired September 2012?

DeleteI'm afraid SM Business Centers do not process changes in a professional's name either by way of marriage or in your case, annulment. I was advised before to process the same at PRC Offices. You may want to visit your PRC Regional Office or check out their website at http://www.prc.gov.ph.

If my memory serves me right, I was not asked to present my expiring license. However, if in case they would be asking you to present it, I am afraid you would need to process your license renewal at PRC offices accompanied by an Affidavit of Loss.

I have the same issue. My license expired april 2012 and i lost it a year after. I will try to renew my id tomorrow. I have a photocopy of my id so i still have all the details i need. I believe the penalty fee is 20 pesos per month from the validity date.

It's good that you have a photocopy of your ID. Again, you just need your REGISTRATION DATE, LICENSE NUMBER and EXPIRATION DATE for filling up the application form.
With regards to the penalty fees, I am not sure as I renewed mine prior to expiry. I tried asking but the teller said that it would be computed based on the number of months, as to how much though, that I don't have an info with me.
Just like what I replied to Ms. Hopeful, I was not asked to present my expiring ID back then when I renewed.

I have checked this concern with SM Business Center back when I processed my renewal and according to the teller, they do not process replacement of lost PICs as they are only contracted for renewals.
I am not quite sure if they have changed their process nowadays. I would suggest better check with your nearest SM Business Center to verify.

Hello sir Eric.I renewed my license on Oct.3,2014 at PRC, Manila.I was told to just check on the PRC website to see if my new ID will be available for release.But,until now there is no announcement regarding available IDs for release (renewal).I also asked the cashier at PRC if the courier service is available, sadly they say NO.My friend told me to see for a RELEASE DATE printed on my claim stub but THERE IS NO such thing on my claim stub.It says on the stub, they are going to dispose off the unclaimed IDs after 2 mos.I tried calling all the PRC hotlines but all were out of service.When do you think can I claim my renewed PRC ID? If you have your ID claim stub with you, it should have a date specified, just below the table where you filled in you details. Specifically, it would say, "Please present this slip to claim your professional ID on __________________ at Window _____________."
You may also want to call PRC at (02) 736-22-48.

PRC might have already given them authority to do so, hence, it would be best if you can check with SM Business Center personally.

To answer your questions:
1. The process of renewing expired PRC ID would be the same as how you would have renewed it prior to expiry. Th only difference would be the amount you need to pay as it has accumulated penalties, of which I do not have the computation.
2. I believe PRC only has one (1) application form/renewal form.
3. An authorization letter or an SPA is only required from your mother once she claims your professional ID card as indicated.

DeleteI believe, yes, you can do both - renewal of your professional ID card and change of status - at the PRC Central Office or Regional Offices. However, aside from the application form, you would also need to submit a duly accomplished and duly notarized "Petition for Change of Registered Name Due to Marriage" which is available for download here, http://www.prc.gov.ph/uploaded/documents/PETITION%20FOR%20CHANGE%20OF%20REGISTERED%20NAME%20DUE%20TO%20MARRIAGE.pdf.
Likewise, since you mentioned that you are out of the country, you can have a representative process the same, provided that you have duly accomplished both application form and petition form.

ReplyDeleteJust have a quick question. Is it true that a relative (my sister in my case), can not process my prc renewal at SM malls even in the presence of an authorization letter? Yes, I read your response that it's because they will take a photo of you, but, that is why you will also enclose a photo together with the authorization letter, renewal form, photocopy of old ID.
It wasn't mentioned in the prc website that renewal can only be made in PRC if the person/professional itself is not present. After all, the reason why we have sm malls is for the convenience of the people renewing.

If I'm not mistaken, since it has been almost two years since I renewed at SM Business Center, they would not allow any representative, or your sister in your case to process renewal of your professional identification card because, as you've read, SM Business Center will take your photo personally. They are not allowed by PRC to receive a picture of the professional.
I understand your sentiment that this information is not in the PRC website. However, since you mentioned that your sister has a photo of you, an authorization letter and a photocopy of your expired PRC ID card, I would strongly recommend that you ask her to process the same at the PRC Central Office or Regional Offices near your area.

To answer your concern, I am afraid that if your mom would renew her PIC in SM Malls, it would take quite some time. I would therefore recommend that she visit the nearest PRC Regional Office in Legaspi to process the same, here's the details of PRC Legaspi City:

DeleteArticle V Section 26 of RA 9173 states that "Nurses who have not actively practiced the profession for five (5) consecutive years are required to undergo one (1) month of didactic training and three (3) months of practicum. The Board shall accredit hospitals to conduct the said training program.”
I would therefore strongly recommend that you go to your nearest PRC Office to inquire about your particular concern.

Hi Sir. Ask ko lang po kasi mali po yung birthdate ko sa prc. Di ko pa po nakukuha license ko kasi kakapasa ko lang po sa board. Ano po bang dapat ilagay sa personal affidavit? And kukunin po ba nila yung license ko na may maling birthdate?

As far as I'm concerned, we are not required to surrender our old ID. Matter of fact, I still have my original ID with me.

DeleteHere are the requirements for renewal of professional ID card:
1. Duly accomplished form. (Can be downloaded here http://prc.gov.ph/uploaded/documents/RD%20renewal%20form%20051012.pdf)
2. Two (2) pcs passport size picture; close up, colored, white background with complete name tag.
In addition, you would need to submit a notarized Affidavit of Loss if your professional ID card is still current.

DeleteAs far as I remember, there is no need to present an affidavit of loss if you are just renewing your professional ID card. You would need to present one if in case of a lost PIC which is still valid and you would be requesting for a duplicate.

Requirement for Inactive Nurses Returning to Practice. — Nurses who have not actively practiced the profession for five (5) consecutive years are required to undergo one (1) month of didactic training and three (3) months of practicum. The Board shall accredit hospitals to conduct the said training program.
But, I have not heard anyone who underwent refresher training. You might just need to pay for the corresponding penalties and charges.

Hello sir,ask lang po,kung hindi ko ba na praktis ang pagtuturo ko for how many years,kelangan ko po bang mag undergo ng mga trainings ang seminars,bago mag apply as teacher?matagal po kasi ako nasa abroad bilang domestic worker at ngayon plano ko pong i pursue ang teaching.thanks sa sagot.
